Volunteering is a great way to support Middletown Elementary School. If you have not completed a Volunteer Records Check Form and you plan to volunteer on a regularly scheduled and/or continuing basis, or if you will have supervisory responsibility for students at a school site or on a school-sponsored trip, please complete the Volunteer Records Check Form and return it to your child's school.
